Who is Eligible

All covered members are insured under the Welfare Fund’s group life insurance policy. 

What is the Benefit

This $10,000 benefit will be paid to your designated beneficiary (or beneficiaries), regardless of the cause of your death.

Designating Your Beneficiary (Or Beneficiaries)

IT IS ESSENTIAL TO NOTIFY THE FUND OFFICE IN WRITING OF YOUR DESIGNATED BENEFICIARY (OR BENEFICIARIES).

You may name any person or persons as beneficiary (or beneficiaries) of your Life Insurance Benefit when completing an Enrollment Card. You may change your beneficiary at any time by filling out a change of beneficiary form. Enrollment Cards and change of beneficiary forms are available directly from the Welfare Fund Office. For your convenience, such forms are often available through your Union delegate at your work location or on our website at www.mightyunionfunds.org 

Should your designated beneficiary die, you should consider the designation of a new beneficiary. If there are no living beneficiaries, or if you have not designated beneficiaries, the Life Insurance Benefit will be paid according to the rules and regulations of the group policy.

In the event of a discrepancy between this outline and the group life insurance policy, the group life insurance policy will govern.

How To Obtain The Benefit

In the event of the death of a covered member, the Fund Office should be notified immediately, so that the Welfare Fund can give prompt attention to the claim.

Your Guaranteed Insurance Protection During Total Disability

If while still insured you become totally disabled at any time prior to age 60, and submit satisfactory evidence of such disability through the Funds’ office to the insurance company, your group life insurance will be continued at no cost to you, as long as you remain totally disabled.

What If I Leave The Welfare Fund? (Your Conversion Privilege)

If you are no longer eligible for Welfare Fund coverage, you may convert your group life insurance, without a medical examination, to any one of a number of individual life policies offered by the underwriting insurance company. If you decide to exercise your conversion privilege, you must do so within 31 days after you are no longer eligible for Welfare Fund coverage. The new policy will be effective at the end of the 31 day period, and the premiums will be the same as you would ordinarily pay if you applied for an individual policy at that time.

If your death should occur during this 31 day period, your group life insurance will be paid whether or not you have applied for an individual policy. If you want to continue this coverage under direct payment, call the Fund office. If you covert, you will receive a certificate of insurance outlining your coverage.
