Who Is Eligible

All covered members are insured under the Fund’s group accidental death and dismemberment insurance policy.

What Is The Benefit

The policy provides the following benefits for the following accidental losses:

For Loss of Life …………………………………………  $10,000*

*Paid to your beneficiary or beneficiaries IN ADDITION TO the $10,000 Group Life Insurance proceeds

For Loss of:

 Both hands:……………………………………………………….$10,000

 Both feet:…………………………………………………………..$10,000

 Sight of both eyes:………………………………………………$10,000

 One foot and sight of one eye:……………………………..$10,000

 One hand and sight of one eye…………………………….$10,000

For Loss of:

 One hand:……………$ 5,000

 One foot:……………..$ 5,000

 Sight of one eye……$ 5,000

The total payment under this policy for any one accident may not be more than the full amount of insurance ($10.000). Payment will be made to you.

Benefit Limitation

The accidental death and dismemberment insurance benefit is not payable for losses caused by war, suicide, attempted suicide, bodily or mental infirmity, disease, medical or surgical treatment, hernia or bacterial infection.

How To Obtain The Benefit

In the event of accidental death or dismemberment, the Fund office should be notified immediately so that the Welfare Fund can give prompt attention to the claim.
